Dialysis Transports

 

If you need Ambulance (stretcher) transportation to and from hemodialysis treatments

A Physician Certification Form for Medical necessity must be signed by the physician ordering the transport for all insurances.

You must be bed confined and non ambulatory

We must have your medical history and the reason it is necessary for you to be transported by ambulance in effect the reason you cannot travel by other means.

Medicaid provides coverage under 50 miles one way ( under 100 miles roundtrip ) without prior authorization

Medicaid managed care insurances must have prior authorization ( Caresource, Molina etc) and we obtain this for you

Most commercial insurance cover, some require prior authorization

Because of the vast variety of plan types per each insurance company it is your responsibility to know whether or not your commercial insurance plan

covers ambulance and non-emergency transportation, it is dependent upon your plan.

We need complete medical history with current medical conditions which make it necessary for you to travel by us.

(Note: Per Medicare they will not provide coverage for transports to see your dialysis physician even if they are in the same clinic. Per Medicare provider assistance their response was

" a physician visit is a physician visit no matter where it occurs we only pay for transports to recieve hemodialysis treatment"

 

If you need Ambulette ( wheelchair van) transportation to and from hemodialysis treatments

Medicare will NOT cover this

Medicaid does and only requires prior authorization if over 50 miles one way- but Medicaid managed care must have prior authorization we obtain this for you

You must be wheelchair confined- nonambulatory

We need an Ambulette Physician Certification of Medical Necessity which we obtain for you

You need to provide complete medical history and current medical conditions.
